MBChB PHASE 2 STUDENTS ONLY: Please use this form for absences due to short-term illness. Forms must be completed before the first missed teaching session starts and you are also required to follow your placement provider's absence reporting process. Medical evidence is required for absences longer than 5 days and should be emailed to the Professional Development team. If your illness means you will miss an assessment you should also use the Mitigating Circumstances process.
PHASE 1 MBChB STUDENTS & STUDENTS ON ALL OTHER COURSES please log your sick absences via your Moodle Course Attendance page. Sickness forms submitted via this link will not be processed. Medical evidence is required for absences longer than 5 days and should be emailed to the Professional Development team.  If your illness means you will miss an assessment you should also use the Mitigating Circumstances process.

Exceptional absence request: for reasons other than illness. Evidence is required for all requests. Unless the situation requiring absence is an emergency (and supported as such by the evidence), this form should be completed at least two weeks prior to the requested absence date.

Partial day absence: This applies where there are two or more teaching sessions scheduled on the same day and absence is being requested from only one of them.
